Upper Blepharoplasty (Hooded Eyelids)
Heavy, hooded upper lids, often caused by a condition known as dermatochalasis (loose, excess eyelid skin), can make your eyes look tired and even shade your vision. Upper blepharoplasty is a precise form of eyelid surgery that removes this excess skin (and small fat pockets if needed) through a fine incision hidden in your natural crease.

There are mainly 3 types of blepharoplasty (also known as eyelid surgery):
- Upper Eyelid (hooded eyes) Surgery :
This procedure involves a discreet incision in the natural fold, removing excess skin and fat for a seamless heal along the eyelid's contour.
- Transconjunctival Lower Eyelid Surgery :
This method involves an internal incision inside the lower eyelid to remove excess fat, avoiding external cuts. While it doesn't remove skin, a laser might be used to tighten the area.
- Transcutaneous (Subciliary) Lower Eyelid Surgery :
This procedure involves a concealed incision beneath the lashes to remove or redistribute excess skin, muscle, and fat, addressing puffiness and ensuring a natural-looking heal within the creases.

During your blepharoplasty consultation, your full medical history is examined to make sure you’re an appropriate candidate.
Then you’ll have an eyelid examination, which will allow your doctor to assess the characteristics of your eyes' tissue.
You’ll also be shown the expected results that you will get after the blepharoplasty surgery.
During your consultation, you are in the driver’s seat. Feel free to ask as many questions as you like. Your doctor will address any concerns you might have so you can be confident about the choice you’re making.
Usually, a blepharoplasty surgery takes between 1 and 2 hours.
Immediately following your procedure, ice packs will be applied to the area in order to reduce swelling. Within a couple of hours you can leave the hospital and go home. You will need somebody to accompany you home as you will not be fit to drive for 24 hours.
Antibiotics, painkillers and sometimes eye-drops are prescribed to you by your surgeon when leaving the hospital. Generally, pain is very minimal after this procedure, and you will usually not require the painkillers after the first 24-36 hours. Bruising and swelling should be expected during the days after surgery.
Within 1 week the bruising should disappear and the swelling will have decreased substantially. You should expect to be fit for social interaction within about 10 days following your procedure. Whilst some swelling will still be present, it is generally quite minimal and will not look too dramatic.
Common temporary effects include swelling, bruising, and irritation or watery eyes, and some people can have dry eyes for a period after surgery.
Risks can include bleeding, infection, scarring, difficulty closing the eyes, eyelid position changes (such as lower lid pulling down or outward turning), and asymmetry.
Rarely, there can be double vision or vision problems, and very rarely serious complications related to bleeding behind the eye.
In most cases, no. The HSE generally does not cover eyelid surgery when it’s being done for cosmetic reasons. It may be considered only when it’s medically necessary, such as when heavy upper eyelids cause documented vision obstruction (often confirmed by visual field testing) or specific clinical conditions.
If you think your eyelids are affecting your sight or comfort, start with your GP for referral into public assessment.
Yes, but in most cases the scars are small and well hidden. For upper eyelid surgery, the incision is usually placed in the natural eyelid fold, and for lower eyelid surgery it may sit just below the lashes or inside the lower lid (so there is no visible external scar with the transconjunctival approach). As the area heals, scars typically fade over time and often become hard to notice.
It addresses hooded or heavy upper lids and also improves under-eye bags and puffiness. The goal is a balanced, refreshed result across the whole eye area.
If you have concerns in both areas, combining can provide a more harmonious improvement. It also means one treatment plan and one recovery period when appropriate.
It can involve more swelling than treating only one area, but most patients still plan for around a week for visible bruising and swelling to settle. Your surgeon will set realistic expectations based on your case.
