Upper Blepharoplasty for Men (Hooded Eyelids)
Heavy, hooded upper lids are often caused by loose excess eyelid skin (dermatochalasis). In men, this can make the eyes look permanently tired and, in some cases, reduce the upper field of vision. Upper eyelid surgery for men is a precise procedure that removes excess skin and, when needed, small fat pockets through a fine incision placed within the natural upper lid crease, so the result stays discreet and natural.
What it does
Restores a cleaner upper lid contour without looking “done”
Reduces heaviness so the eyes look more open and refreshed
Can improve upper-field vision when skin was overhanging
- Best for: hooded lids, skin resting on the lashes, a heavy upper lid look, a tired appearance.
- Scars: hidden in the natural crease.
- Results: lighter lids, sharper definition, younger masculine look.
Lower Blepharoplasty for Men (Under-Eye Bags)
Lower blepharoplasty is a precise procedure designed to reduce under-eye bags and puffiness so you look more rested. In men, the goal is to smooth the under-eye area while keeping a natural, masculine eye shape. Surgery is usually done through a hidden incision inside the lower lid (transconjunctival) or just below the lash line (transcutaneous) to reshape or reposition fat and, when needed, tighten excess skin.
What it does
Reduces under-eye bags by reshaping or repositioning fat pads
Smooths creasing and loose under-eye skin for a cleaner contour
Improves the lid-to-cheek transition so the under-eye looks fresh, not hollow
- Best for: under-eye bags, persistent puffiness, deep tear trough shadows.
- Results: flatter under-eye area, smoother lid–cheek blend, reduced shadowing.
Why Should You Combine Upper and Lower Eyelid surgeries?
Combined blepharoplasty for men treats the upper and lower eyelids in a single procedure. It removes excess upper-lid skin and repositions or reduces lower-lid fat to smooth eye bags, giving a more rested, refreshed look in one surgical session.
- One tailored plan for the full eye area
- One recovery period when appropriate
- Better harmony between upper lids and under-eye contour

PHASE 1
IMMEDIATE POST-SURGERY PERIOD
• Recovery time from eyelid surgery for men will vary amongst different individuals.
• Directly following your surgery, you should rest with your head elevated.
• You may be instructed to apply a cold compress to your eyelids.
PHASE 2
FIRST FEW DAYS AFTER SURGERY
• Plan time off: many people take about 1–2 weeks, depending on work and procedure.
• Tightness, mild discomfort, swelling and bruising are normal early on.
• Typically, bruising and swelling disappear within 10-14 days, but can last 2–3 weeks.
• Vision may appear blurry for a few days, with increased sensitivity to lights.
• Symptoms like tearing, dryness, burning, or itching feelings may accompany.
PHASE 3
POST-SURGERY CARE FOR MONTHS
• Protect the area from sun (sunglasses, hat, and SPF once incisions are closed). UV can worsen swelling and darken healing scars, and protection may be advised for many months.
• Consider purchasing larger frames to help cover up all areas around your eyelids.
• Sun exposure can make your skin look pinkish, red, or splotchy and will harm your sensitive eyes.

✦ You want a subtle refresh that keeps a natural, masculine eye shape.

Male Eyelid Surgery FAQs
Male eyelid surgery is a procedure in which excess skin and fat are removed from the upper and lower eyelids, correcting drooping upper eyelids and removing puffy bags from below the eyes, to give a well-rested, refreshed appearance.
There are mainly 3 types of blepharoplasty (also known as eyelid surgery):
- Upper Eyelid (hooded eyes) Surgery :
This procedure involves a discreet incision in the natural fold, removing excess skin and fat for a seamless heal along the eyelid's contour.
- Transconjunctival Lower Eyelid Surgery :
This method involves an internal incision inside the lower eyelid to remove excess fat, avoiding external cuts. While it doesn't remove skin, a laser might be used to tighten the area.
- Transcutaneous (Subciliary) Lower Eyelid Surgery :
This procedure involves a concealed incision beneath the lashes to remove or redistribute excess skin, muscle, and fat, addressing puffiness and ensuring a natural-looking heal within the creases.

During your blepharoplasty consultation, your full medical history is examined to make sure you’re an appropriate candidate.
Then you’ll have an eyelid examination, which will allow your doctor to assess the characteristics of your eyes' tissue.
You’ll also be shown the expected results that you will get after the blepharoplasty surgery.
During your consultation, you are in the driver’s seat. Feel free to ask as many questions as you like. Your doctor will address any concerns you might have so you can be confident about the choice you’re making.
Usually, a blepharoplasty surgery takes between 1 and 2 hours.
Immediately following your procedure, ice packs will be applied to the area in order to reduce swelling. Within a couple of hours you can leave the hospital and go home. You will need somebody to accompany you home as you will not be fit to drive for 24 hours.
Antibiotics, painkillers and sometimes eye-drops are prescribed to you by your surgeon when leaving the hospital. Generally, pain is very minimal after this procedure, and you will usually not require the painkillers after the first 24-36 hours. Bruising and swelling should be expected during the days after surgery.
Within 1 week the bruising should start to disappear and the swelling will have decreased substantially. You should expect to be fit for social interaction within about 10 days following your procedure. Whilst some swelling will still be present, it is generally quite minimal and will not look too dramatic.
Common temporary effects include swelling, bruising, and irritation or watery eyes, and some people can have dry eyes for a period after surgery.
Risks can include bleeding, infection, scarring, difficulty closing the eyes, eyelid position changes (such as lower lid pulling down or outward turning), and asymmetry.
Rarely, there can be double vision or vision problems, and very rarely serious complications related to bleeding behind the eye.
In most cases, no. The HSE generally does not cover eyelid surgery when it’s being done for cosmetic reasons. It may be considered only when it’s medically necessary, such as when heavy upper eyelids cause documented vision obstruction (often confirmed by visual field testing) or specific clinical conditions.
If you think your eyelids are affecting your sight or comfort, start with your GP for referral into public assessment.
Yes, but in most cases the scars are small and well hidden. For upper eyelid surgery, the incision is usually placed in the natural eyelid fold, and for lower eyelid surgery it may sit just below the lashes or inside the lower lid (so there is no visible external scar with the transconjunctival approach). As the area heals, scars typically fade over time and often become hard to notice.
No, male blepharoplasty isn't exactly the same as for women, while the basic surgical steps (removing skin/fat) are similar, the approach differs significantly, focusing on preserving masculine features like a naturally lower brow and heavier eyelids, avoiding the wider, more rounded look often desired in women, and accounting for men's thicker skin and muscle for a naturally refreshed, not feminized, appearance.
It addresses hooded or heavy upper lids and also improves under-eye bags and puffiness. The goal is a balanced, refreshed result across the whole eye area.
If you have concerns in both areas, combining can provide a more harmonious improvement. It also means one treatment plan and one recovery period when appropriate.
It can involve more swelling than treating only one area, but most patients still plan for around a week for visible bruising and swelling to settle. Your surgeon will set realistic expectations based on your case.
The aim is rejuvenation, not changing your identity. The plan is designed to look natural and suit your facial features.
Yes. Combined eyelid surgery for men is planned to maintain a natural, masculine look.
