Open vs Closed Rhinoplasty

Closed Rhinoplasty

Closed rhinoplasty is performed with incisions placed inside the nostrils, which means there is usually no visible external scar. It is often suitable when the changes are more straightforward and the surgeon can achieve the desired refinement without needing full exposure of the nasal framework.

Open Rhinoplasty

Open rhinoplasty includes a small incision across the columella, the tissue between the nostrils, in addition to internal incisions. This approach gives the surgeon clearer access and visibility, which can be helpful for more complex reshaping, detailed tip work, significant asymmetry, or revision cases. In most patients, the small external scar typically fades well over time.

rhinoplasty

The best technique depends on your nasal structure, the result you want, and whether functional correction such as septal work is also needed.

You may be a good candidate for rhinoplasty if you are in good general health, your facial growth is complete, and you have clear, realistic goals for how you want your nose to look or function. You’re usually a better fit if your weight is stable, you don’t smoke or vape (or you can stop as advised), and your expectations are focused on natural improvement rather than “perfection.” Rhinoplasty can be considered for concerns like a hump, wide bridge, bulbous or droopy tip, crooked nose, or breathing issues caused by structural problems. The best way to confirm suitability is a consultation where your surgeon assesses your nasal structure, skin thickness, and breathing function to recommend the safest plan.

Depending on the case, we use either a “closed” technique (incisions inside the nose with no visible scars) or an “open” technique (incision at the base of the nose with a small, barely noticeable scar).

Most surgeries are performed under general anesthesia and last between 1 and 3 hours.

Rhinoplasty (or nose reshaping) can change the overall shape and size of the nose, reduce or increase the nasal dorsum, adjust the tip, alter the nasolabial angle, refine the nostrils, and enhance the overall appearance of the nose. Each treatment is customised to individual needs, guided by a cosmetic surgeon who will advise on the possibilities and limitations specific to you.

Rhinoplasty usually takes about 1 to 3 hours, depending on what needs to be changed and whether it’s a primary or revision case.

Most patients need up to 2 weeks off work. A splint is often removed at around 1 week, bruising and swelling usually improve over the next 2 to 3 weeks, and strenuous exercise is typically avoided for 4 to 6 weeks. Final refinement takes time, and swelling can continue to settle for months (up to around 6 months).

Rhinoplasty is a commonly performed operation and is generally safe when done by a qualified surgeon in an appropriate surgical setting. However, it is still major surgery, so you should expect a full pre-op assessment and discussion of risks based on your individual health.

Sometimes. In a closed rhinoplasty, incisions are hidden inside the nostrils, so there is usually no visible external scar. In an open rhinoplasty, there is a small incision across the columella (the tissue between the nostrils) which can leave a small scar that typically fades over time.

Risks can include bleeding, infection, and anaesthesia reactions. Procedure-specific risks may include breathing difficulties, poor wound healing or scarring, asymmetry, and persistent swelling or numbness.

Open rhinoplasty uses an incision across the columella (the tissue between the nostrils), allowing the surgeon clearer access for more complex reshaping. Closed rhinoplasty places incisions inside the nostrils, so there’s usually no visible external scar and it’s often used for more limited changes.

You’ll usually see an early change once the splint comes off (often around 1 week), but swelling continues to settle for months. Most swelling improves within the first 3 months, while subtle swelling, especially in the tip, can take up to 12 months to fully resolve, which is when final results are clearest.

Most people do not have a lasting voice change, but some notice a temporary difference early on due to swelling and nasal congestion affecting airflow. Changes are usually subtle and not problematic for speech, though people who use their voice professionally should discuss this with their surgeon during their consultation.
